Environment At Oak Street Health, you’ll feel empowered to build relationships with your patients - allowing you to provide better care and helping them live healthier
While the typical primary care panel in the U.S. is around 2,500 patients for a single physician, our panels at Oak Street Health are around 500-750 for a team. This creates an opportunity to spend more time with patients, build deeper relationships, and to better execute/coordinate care plans.
We’re rebuilding healthcare as it should be. Since our founding in 2012, our mission has been to build a primary care delivery platform that directly addresses rising costs and poor outcomes, two of the most pressing challenges facing the United States healthcare industry.
