CHA is recruiting a Senior Medical Director of Population Health Management Outcomes. As an integral part of the Population Health Management leadership team the incoming Senior Medical Director will provide oversight and guidance for population health performance goals and objectives.
Ideal candidates will be Board Certified in Family Medicine, Internal Medicine, or Internal medicine/Pediatrics and have 3+ years of experience in a clinical leadership role. Candidates should possess a strong desire to provide the highest quality care to our diverse patient population.
