Christiana Care Health System (CCHS), headquartered in Wilmington, Delaware, is a not-for-profit, non-sectarian health system that includes two hospitals with more than 1,100 patient beds, a home health care service, preventive medicine, rehabilitation services, a network of primary care physicians and an extensive range of outpatient services. It is recognized as a regional center for excellence in cardiology, cancer and women’s health services and is home to Delaware’s only Level I trauma center for adults and children and a Level III neonatal intensive care unit. The Service Line Physician Leader of Primary Care and Community Medicine is a member of the CCHS leadership team who oversees quality, safety and population health management initiatives within primary care and community medicine. He/She will oversee the delivery of care across 15 practices that include 120 clinicians (physicians, physician assistants and nurse practitioners) who care for more than 120,000 patients per year. It is estimated that within three years, the patient complement will increase to 275,000; therefore, the strategic plan calls for the addition of 80 primary care clinicians over the next two years. This highly visible leader will organize and drive service line efforts to optimize clinical operations and ensure that the care delivered is effective, efficient and innovative. The Service Line Physician Leader focuses service line work to support the development of highly reliable processes that reduce unnecessary clinical variation and meet evidence and consensus-based standards of care.
